Differentiate Depression from Dementia
Depression Dementia
bulletRelatively rapid onset
bulletDepressive symptoms start before dementia
bulletPatient complains more than family
bulletAppears depressed
bulletResponse of "I don't know"
bulletInconsistent Cognitive impairment
bulletResponse to antidepressant
bulletvery slow onset
bulletdepressive symptoms develop after cognitive decline
bulletPatient tends to deny any memory problem
bulletMay or may not appear depressed
bulletTry to give an excuse or general answer
bulletCognitive impairment fairly consistent
bulletAntidepressant may have no effect

Causes of Dementia
Alzheimer's dementia
Multiinfarct dementia
Normal Pressure Hydrocephalus
bullet3 main symptoms: Dementia, Gait Apraxia, Incontinence
bulletLanguage functions preserved
bulletMost common cause of gait abnormality plus Dementia is multiinfarct dementia
bulletMay progress over months to years then reach a plateau
bulletMRI shows large ventricles
bulletLP may result in temporary improvement
bulletTreatment is VP or LP shunt
HIV dementia
bulletYounger patient
bulletMemory loss
bulletFrontal lobe dysfunction, personality change, social withdraw

bulletMay be the initial presentation of AIDS

bulletAlcohol or drugs, Vitamin deficiencies, Hormonal disturbances.

bulletPrimary progressive Aphasia:
bulletprogressive aphasia without true dementia
bulletJacob Creutzfeld Disease:
bulletprogressive dementia with seizures, myoclonus, ataxia, visual disturbance, motor neuron dysfunction
bulletChronic infections, vasculitis:
bulletCryptococcal, fungal.
bulletProgressive multifocal leukoencephalopathy
bulletBilateral Subdural hematoma
bulletBrain tumor: especially frontal glioma
bulletNeurodegenerative Disorders
bulletParkinson's disease
bulletLewy body dementia
bulletProgressive supranuclear palsy
bulletFrontotemporal dementias (e.g., Pick's disease, primary progressive aphasias)
bulletCortical-basal degeneration
bulletHippocampal sclerosis
