Restless Leg Syndrome
Overview
bulletUncomfortable feeling in the legs
bulletPredominantly when the patient is supine.
bulletDifficulty in describing the exact nature of the discomfort: ache, burning, crawling sensation
bulletMay involve the arms and trunk
Diagnosis
bulletLower extremity paresthesias and motor restlessness
bulletAggravated by nightfall and rest   
bulletRelieved by standing and walking around.
bulletSleep is often disturbed.
bulletResting dyskinesias while awake   
bulletPeriodic movements in sleep
Exam
bulletNormal neurologic examination
Testings
bulletDiagnosis: based on clinical criteria
bulletLaboratory testing: anemia, B12, ferritin, and folate levels
bulletEMG: done to evaluate for neuropathy
bulletSleep Lab: polysomnography
Treatment
bulletMedication treatment:
bulletStart at low dose hs & titrate.
bulletPatient may become tolerant to medications.
bulletMay be necessary to rotate 2 or 3 drugs every 1-2 months.
bulletLevodopa: start with half of 25/100 tablet qHS and titrate up.
bulletDopamine agonists:
bulletPergolide (Permax) 0.05-1.0 mg
bulletBromocriptine (Parlodel) 1.25-10 mg
bulletPramipexole (Mirapex) 0.125-4.5 mg
bulletRopinirole (Requip) 0.25-24 mg
bulletBenzodiazepine:
bulletClonazepam (Klonopin) 0.25-1.5 mg
bulletDiazepam 2-15 mg
bulletOpioids:
bulletPropoxyphene (Darvocet) 50-200 mg
bulletCodeine 30-60 mg
bulletOther:
bulletGabapentin (Neurontin): 300 - 600 mg
bulletClonidine (Catapres) 0.1-0.3 mg
bulletBaclofen (Lioresal) 10-80 mg
bulletCarbamazepine (Tegretol) 200-600 mg
